All entries to the Fit Mother Project Veterans Transformation Contest 2021 should be submitted no later than Sunday 31st January 2021.
PLEASE NOTE: Any entries submitted after this date will not be eligible for entry into the contest. This decision is final.
Entry Submission:
To officially enter the Fit Mother Project Veteran Transformation Contest 2021, you will need to complete the registration form, at https://forms.gle/cezMykXTFyXJjLEB9, supplying all necessary information requested and providing your transformation journey video in the proper format.
Fit Mother Project, Fit Mother Project LLC, and AMB Wellness Partners LLC, or any of their partners will not be held responsible for internet connectivity, application technical issues, misplaced or lost entries.
Prizes:
The prizes for the FMP Veteran Transformation Contest 2021 will be:
- 1st Place: $2,000 USD
(If this prize is won outside of the USA, then a wire payment will be sent for the USD amount set forth above. All transaction, conversion, applicable taxes, and/or other additional fees will be the responsibility of the recipient)
Eligibility:
To be eligible to enter the Fit Mother Project Veteran Transformation Contest 2021 and win any of the prizes indicated above, you must be over 18 years of age, be a customer of the Fit Mother Project, and have fully completed the registration form and submitted a qualifying video no later than 31st January 2021.
Video Submission:
All videos should be submitted in MP4 or .MOV file format, or via a YouTube link or link to other publicly accessible video submission platform. The video should be taken in good lighting conditions, be clear and in high definition, be taken against a plain background, and be taken purposefully for submission to this contest. It is a good idea to hold an image of (or edit into the video) your before and after pictures.
Your video should outline your achievements and contain your before and after statistics, your thoughts about the program(s), what the journey has helped you overcome, and how you feel now compared to when you first started. You can include any other information you feel would be beneficial for the contest.
Use of Videos, and Written Material:
By submitting your video, other files, and any written information, you give full permission for Fit Mother Project, Fit Mother Project LLC, AMB Wellness Partners LLC, or any and all affiliated partners to use them (in whole or in part) for promotional, advertising, and marketing purposes in any media and in any manner whatsoever, without limitation or restriction or further consideration of any kind.
All information and materials submitted throughout the contest become the sole and exclusive property of Fit Mother Project LLC and its parent Company, AMB Wellness Partners LLC, and such consent is deemed given by your entry and participation in the contest.
Judging:
Judging will be based on 2 main criteria.
- Overall body transformation throughout your journey
- Overall transformation story (Detailed in your video submission)
PLEASE NOTE: This is not solely a weight loss contest, but rather a transformation contest. The winner will not be selected by how much weight they lost, but by how their body transformed and their life changed throughout the process.
The judging will be completed by the Fit Mother Project Team, and a FaceBook poll within the private FM30X Facebook groups. Their agreed decision will be final.
Rules & Regulations:
Fit Mother Project LLC and AMB Wellness Partners LLC reserve the right to interpret these rules and, if necessary, to amend the rules at their sole discretion without notice to individual contest participants. Any amendments to the rules or changes in prizes will be published online at https://www.fitmotherproject.com.
Fit Mother Project, Fit Mother Project LLC, and AMB Wellness Partners LLC shall have the sole right in its sole discretion to disqualify any contest participants without explanation for any reason at any stage of the contest, and Fit Mother Project and AMB Wellness Partners LLC shall be fully and completely released and discharged from any liability or responsibility in this regard.
Each participant of the Fit Mother Project Veteran Transformation Contest 2021 assumes all risk of injury, harm, or loss of any kind arising from participation in the contest.
If you are not experienced with strength training, or if you are changing your exercise program, consult with a qualified trainer or coach. Participants expressly release Fit Mother Project and AMB Wellness Partners LLC and affiliated companies, and the directors, officers, and employees of any of them from all risk, loss, injury, damage, or harm that may arise from participating in the contest.
Participants must notify Fit Mother Project and AMB Wellness Partners LLC in writing of any change of email address or phone number by emailing customer success at [email protected]. Winners who cannot be reached within ten (10) days at the email address or phone number submitted through the application will be disqualified.
As individuals differ, their results will differ, even when using the same program.
Submission of your video, measurements, and written forms shall be deemed your acceptance of these Rules and Regulations (as may be amended) and your voluntary transfer to Fit Mother Project and AMB Wellness Partners LLC of all right, title, and interest, including copyright, of your video and written responses.
General Disclaimer and Release of Liability:
In acceptance of a participants entry into the contest and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, each participant hereby takes action for themselves, their executors, administrators, agents, representatives, spouse, children, heirs, next of kin, successors, and assigns, and acknowledges and agrees as follows:
(a) to be bound by the Official Rules and decisions of Fit Mother Project, AMB Wellness Partners LLC, and any and all affiliated partners in any way related to the Contest;
(b) that participation in the Contest is voluntary;
(c) The participant is not a minor;
(d) The participant is of sound mind and body and has not been advised otherwise by a qualified medical person;
(e) The participant possesses all rights and faculties necessary to execute, deliver, and perform this agreement;
(f) certifies that the participant meets all eligibility requirements as set forth in the above ‘Eligibility’ section, which the participant represents they have read, understood and agree to;
(g) certifies that all videos the participant submits for the contest will depict only the participant and do not require the consent of any other third party rights holder
before use;
and (h) The participant is fully aware and assumes the risk of illness, accidents, injury,
death, personal injury, bodily harm, emotional stress, psychological harm, property damage, and all dangers potentially arising from or in connection with participation in the contest.
Further, the participant irrevocably grants to Fit Mother Project, AMB Wellness Partners LLC, and any and all affiliated partners the right to edit, modify, and/or publish and copyright any and all images, videos, and written material submitted throughout the contest.
Fit Mother Project, AMB Wellness Partners LLC, and any and all affiliated partners do not make any warranty, representation, or guarantee, express or implied, in fact, or in law, relative to the use of any prize, including, without limitation, quality, merchantability, and/or fitness for a particular purpose.
Fit Mother Project, AMB Wellness Partners LLC, and any and all affiliated partners are not responsible for any incorrect or inaccurate entry information, human error, technical malfunction, failures, omission, interruption, deletion, or defect of any telephone network, computer, online systems, computer equipment, servers, access providers, or software, including any injury or damage to any participants or any other person's computer relating to or resulting from participation in this contest; inability to access the entry website or any pages thereof; theft, tampering, destruction, or unauthorized access to, or alteration of entries.
Entry submissions that are processed late or incorrectly, or are incomplete, garbled, or lost due to computer or electronic malfunction or traffic congestion on the Internet or any website.
Proof of entering information on our or any website is not considered proof of delivery or receipt. All dates set forth in these rules are approximate.
NOTWITHSTANDING ANY PROVISION OF THESE CONTEST RULES TO THE CONTRARY, FIT MOTHER PROJECT, AMB WELLNESS PARTNERS LLC, AND ANY AND ALL AFFILIATED PARTNERS SHALL NOT BE LIABLE TO PARTICIPANTS OR ANY THIRD PARTY FOR ANY INDIRECT, CONSEQUENTIAL, SPECIAL, PUNITIVE, EXEMPLARY OR INCIDENTAL DAMAGES (INCLUDING DAMAGES FOR LOST PROFITS), EVEN IF THEY HAVE BEEN MADE AWARE OF THE POSSIBILITY OF SUCH DAMAGES.
Fit Mother Project, AMB Wellness Partners LLC, and any and all affiliated partners reserve the right to terminate, withdraw, or amend the Contest and these Official Rules for any reason.
The invalidity or unenforceability of any provision of these Official Rules shall not affect the validity and enforceability of any other provision. In the event that any provision is determined to be invalid or otherwise unenforceable or illegal, these Official Rules shall otherwise remain in effect and shall be construed in accordance with its terms as if the invalid or illegal provision were not contained herein.
Governing Law; Jurisdiction:
This Contest will be governed by and construed in accordance with the laws of the state of Arizona without regard to conflict of law principles. Any controversy arising under, in connection with, or in any way relating to this Contest shall be adjudicated before a state or federal court of competent jurisdiction located in Phoenix, Arizona, U.S.A.
Participant and Fit Mother Project, Fit Mother Project LLC, AMB Wellness Partners LLC, and any and all affiliated partners (i) accept, generally and unconditionally, the exclusive jurisdiction of such court and any related appellate court, and irrevocably agrees to be bound by any judgment rendered thereby in connection with this Contest,
and (ii) irrevocably waives any objection it may now or hereafter have as to the venue of any such suit, action or proceeding brought in such a court or that such court is an inconvenient forum.
